Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
Do I have mental health insurance benefits?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
What is the coverage amount per therapy session?
Is approval required from my primary care physician?

If you do not see your insurance carrier listed above, I may be an Out-of-Network Provider for your particular policy. A representative with your insurance company should be able to detail any out-of-network benefits your policy allows. Call the 800- number listed on the back of your personal insurance identification card for more information.

Payment
All major credit cards (Amex, Discover, Mastercard and Visa) are accepted for payment. All payments are due at the time services are rendered.
